What Is Tracheostomy Care?

A tracheostomy is a surgical opening in the neck that creates a direct airway to the trachea. People living with a tracheostomy — whether temporary or permanent — require specialist nursing care to maintain a safe, patent airway, prevent complications, and manage all associated clinical needs.

Without properly trained nurses in place, a tracheostomy patient cannot safely leave hospital and return home. Ambition24direct's registered nurses enable that transition — keeping patients out of hospital beds, in their own homes, with their families.

Why it matters: Tracheostomy care is one of the highest-acuity interventions in homecare. Clinical Assessment

Our clinical lead reviews the patient's tracheostomy type, suctioning frequency, ventilator status, tube change protocol, and all associated care needs. We establish the staffing model — whether visits, waking nights, or 24-hour nursing.

Staff Selection & Specific Training

We select staff based on existing tracheostomy competency and the specific patient requirements. All team members complete client-specific training and competency sign-off before commencing care — no exceptions.

Care Plan and Emergency Protocol

A written care plan is agreed with the patient, family, and clinical team. This includes the emergency protocol for acute airway events — who to call, what to do, and the escalation pathway to the relevant hospital team.

Ongoing Clinical Oversight

A named clinical lead oversees the package — attending MDT meetings, reviewing care plans, signing off competencies, and remaining accessible to the family and commissioner at all times. Available 24/7.

  • Yes. We provide 24-hour tracheostomy care packages including waking night nursing where respiratory status or suctioning frequency requires it. The model — visiting, live-in, or rotating shifts — is determined by the clinical assessment.

  • Yes, where the clinical protocol authorises it and the nurse has been signed off as competent. Routine tube changes can be carried out at home by a trained nurse according to the patient's care plan. Emergency tube changes are covered by all our tracheostomy-competent nurses as a non-negotiable baseline.

  • Every tracheostomy package includes a written emergency protocol specific to the patient. Our nurses are trained in emergency airway management including emergency tube change. The protocol covers escalation to the relevant hospital team and 999 guidance. We also provide families with clear guidance on what to do if an emergency occurs when a nurse is not physically present.

  • Yes. We provide both adult and paediatric tracheostomy homecare, with separate clinical training pathways for paediatric tube types, suction volumes, and age-appropriate protocols. Paediatric packages are also supported in school and community settings.
