Muscular Dystrophy Care at Home
Muscular dystrophies are a group of inherited conditions causing progressive muscle weakness. For the most severe types — particularly Duchenne MD — clinical needs become highly complex as the condition progresses: full-time wheelchair use, respiratory insufficiency requiring NIV or invasive ventilation, and enteral feeding as swallowing muscles weaken.

Respiratory management is the critical clinical risk in advanced DMD. Nocturnal hypoventilation, acute respiratory infections, and secretion management require a team that is clinically trained — not just aware — of the respiratory risks in MD. We do not treat NIV management as a background task.

Yes. Nocturnal NIV is a cornerstone of respiratory management in advanced DMD and other MD types. We provide waking night nurses trained in BiPAP management, mask fitting, alarm response, and escalation protocols — working in coordination with the respiratory team and neuromuscular service. Nocturnal hypoventilation monitoring is included in the clinical care plan.

Yes. Mechanical insufflation-exsufflation (MI-E) devices — including the CoughAssist — are managed by our nurses as part of routine airway clearance for people with MD. Technique, settings, and frequency are agreed with the respiratory physiologist and neuromuscular team and incorporated into the individual care plan.

Advanced DMD and other severe MD types frequently meet the primary health need threshold for NHS Continuing Healthcare — particularly where respiratory support, enteral feeding, and high-dependency physical care are required. We can provide clinical documentation to support CHC assessments and work with ICBs through the process.

Yes. Transition is a particularly critical period for young men with DMD. We work with Children's Continuing Care and Adult CHC teams, alongside adult neuromuscular services, to ensure continuity of care — keeping the same team wherever possible and updating care plans to reflect the adult service framework and any changes in clinical status.
