Dying at Home — With Clinical Support
The majority of people express a preference to die at home. Yet without adequate clinical support in place, families are forced into hospital admissions that no one wanted — often in the final hours of life.
Ambition24direct provides the clinical nursing support that makes home-based death possible — syringe driver management, symptom control, overnight nursing, and coordination with the specialist palliative care team. We are not a hospice at home service. We are a nurse-led clinical provider that works alongside hospice teams, Macmillan nurses, and specialist palliative consultants to deliver the clinical component of home-based end of life care.

Clinical Assessment
We conduct a rapid telephone or in-person assessment — reviewing the individual's current clinical status, symptom burden, preferred place of death, and family support. We confirm what nursing model is needed and what we can put in place and when.
Care In Place
For end of life Fast-Track packages, we aim to have care in place within 24 hours. We liaise directly with the GP, palliative team, and pharmacy to ensure anticipatory medications are prescribed and available before nursing commences.

Yes. Fast-Track CHC for rapidly deteriorating or end of life patients is a referral pathway we handle 24 hours a day. Call 0330 678 3014 and state it is a Fast-Track referral — our on-call clinical lead will engage directly. We work with the ICB to confirm the package while getting nurses in place simultaneously where the clinical situation requires it.

Yes. Syringe driver setup, monitoring, and medication changes are core competencies for our palliative care nursing team. We work with the prescribing GP or palliative team to ensure the correct anticipatory medication and dose ranges are prescribed and available, and we manage the driver in accordance with the clinical prescription.

Yes — and this is the model we actively encourage. We provide the nursing hours and clinical interventions; the specialist palliative team provides the clinical leadership on symptom management and medication. We attend MDT meetings, communicate clearly with all teams involved, and do not operate in clinical isolation.

Yes. Overnight nursing — whether waking nights or sleeping nights — is available for end of life packages. This reduces family exhaustion during an incredibly difficult period and ensures clinical safety through the night. We discuss the appropriate overnight model during the clinical assessment.

Yes. Paediatric palliative and end of life homecare is available through our children's complex care service — working alongside children's hospices, paediatric palliative care teams, and community children's nursing. We accept urgent referrals from Children's Continuing Care teams 24/7.
